No separate form is prescribed for filing a complaint before the CGRFs or an appeal before the Vidyut Ombudsman.

However, if you want to approach the Hon’ble APERC to file a petition before it, this is the format in which your petition should be and here is the checklist through which it will be put through before being accepted by the Commission for hearing.

The fee that needs to accompany your application before the Commission is mentioned in Regulation 1 of 2005 as amended by Regulation 11 of 2013.
